Washington, D.C. Skilled Nursing Facility is a 296-bed dually certified skilled nursing and long-term care facility located in Washington, D.C. on the border between the Deanwood and Hillbrook neighborhoods. The facility, operated by TL Management LLC treats multiple clinically complex diagnoses, including congestive heart failure, decubitus ulcers, infectious diseases, drug-resistant infections, multi-symptom complications, pulmonary and trachea disorders, and uncontrolled diabetes. Additionally, the facility offers rehabilitative therapy services, including physical therapy, occupational therapy, and speech therapy through its on-site rehabilitation company, The Operator also sub-leases space to a third party provider, DaVita, for dialysis care.

Originally constructed in 1983, the building consists of 133,706 square feet spread over five floors and is situated on 1.44 acres of land. The building has since undergone significant renovations in order to provide its residents with a state-of-the-art therapy and rehab experience in a high quality nursing facility setting. Such renovations include beauty parlor renovations, flooring installation, general design and renovations for resident rooms, installation of indoor cameras in the lobby, kitchen renovation, as well as lounge and corridor updates. The facility has seen approximately $1.2 million in renovations since 2013 and approximately $4.5 million in total since the facility was acquired by TL in 2010. In terms of layout, the building primarily utilizes the 2nd and 3rd floors for long-term and short-term care and the 4th and 5th floors for long-term care with a focus on patients with dementia related diagnoses.

Situated in northeast Washington, D.C., the Deanwood neighborhood is one of the oldest neighborhoods in the city.
