Athens Long-Term Acute Care Hospital is a single-story, 31,000-square-foot, Class A single-tenant medical facility in Athens, Georgia. Located at 775 Sunset Drive, Athens Long-Term Acute Care Hospital is approximately one mile from Athens Regional Medical Center, the region’s leading healthcare provider and a significant source of referral patients. Athens Long-Term Acute Care Hospital is the only long-term acute care hospital serving the more than one million residents of northeast Georgia.

Long-term acute care hospitals are extended-stay specialty hospitals for chronically ill and rehabilitation patients. The average length of stay is 25 days or more.

Built in 2008, Athens Long-Term Acute Care Hospital is leased by Landmark Real Estate Holdings, LLC, which signed a long-term lease through 2025.

Cape Girardeau Long-Term Acute Care Hospital is a single story, 26,000-square-foot, Class A single tenant healthcare facility in Cape Girardeau, Missouri. Located approximately 115 miles to the south of St. Louis and 170 miles to the north of Memphis, Cape Giradeau Long-Term Acute Care Hospital is within one-and-a-half miles of the 258-bed St. Francis Medical Center, the region’s largest acute care facility and a significant source of referral patients. Construction of a 208,000-square-foot Heart Hospital and Cancer Institute is currently underway at the medical center.

Built in 2006, Cape Girardeau Long-Term Acute Care Hospital is 100 percent leased through 2025 by Landmark Holdings of Missouri, LLC, a leading regional operator of long-term acute care hospitals.

 

Columbia Long-Term Acute Care Hospital a single-story, 31,000-square-foot, Class A single-tenant medical facility in Columbia, Missouri, midway between St. Louis and Kansas City. The hospital is located less than a mile away from Boone Hospital Center, a 394-bed full service medical facility, and approximately three miles away from University of Missouri Women’s and Children’s Hospital, a 189-bed general acute care medical center. Both facilities serve as significant sources of referral patients to Columbia Long-Term Acute Care Hospital.

The hospital is leased through January 2026 by Landmark Holdings of Missouri, LLC.

Joplin Long-Term Acute Care Hospital is a single-story, 26,000-square-foot, Class A single-tenant medical facility in Joplin, Missouri.

Located at 2040 West 32nd Street, at the heart of the "Four State Area" where Missouri, Oklahoma, Arkansas and Kansas converge, Joplin Long-Term Acute Care Hospital is less than one mile from St. John’s Regional Medical Center, the region’s leading healthcare provider and a significant source of referral patients.
